Whole-Person Care for Those Who Need It Most

Established in March 2020 and funded through the SAMHSA Certified Community Behavioral Health Clinic (CCBHC) grant, Bridgeway Health Clinic was created to serve as a lifeline for some of our community’s most vulnerable individuals. Our mission is to provide comprehensive, integrated care to those who often fall through the cracks—approximately 2,000 Medicaid and uninsured patients each year—many of whom are experiencing severe and persistent mental illness (SPMI), homelessness, and co-occurring physical health conditions.

Studies show that individuals with SPMI die, on average, 20 years earlier than the general population—largely due to lack of access to appropriate medical care. Bridgeway Health Clinic is uniquely positioned to close this gap. Unlike traditional healthcare systems, we are designed to meet the needs of high-acuity populations through a fully integrated model that combines primary and behavioral health care under one roof.

Our Services

Bridgeway Health Clinic offers a wide range of services to ensure continuity, accessibility, and quality of care:

• Care Coordination and Case Management

We work in close collaboration with Bridgeway Center’s Psychiatric Medicine team to ensure that every patient receives whole-person, wraparound care. This integrated approach removes barriers and supports long-term recovery, health stabilization, and improved quality of life for individuals living with complex challenges.

Continuity of Care

In recent years, the clinic has absorbed many patients formerly served by the HOPE Clinic and Crossroad Clinic, ensuring continued access to care for individuals at high risk of losing critical services. While our application for FQHC Look-Alike status was not approved due to patient volume thresholds, Bridgeway Health Clinic remains a vital component of the regional behavioral health safety net.
